The Community Health Systems Foundation welcomes grant applications from non-profit organizations working to enhance the health, wellness, and quality of life in the following communities: Attleboro, Dighton, Easton, Franklin, Foxboro, Mansfield, Norton, North Attleboro, Plainville, Rehoboth, Seekonk, Taunton, and Wrentham.

The 2025 application form, guidelines, and budget template will be posted soon.
